Formal assessment report

This custom report provides a list of users and their completion status for formal assessments attached to topics. This includes the topic details, such as the topic name and ID, and the assessment details, such as the assessment ID, the user's completion date, raw score, current assessment status (Started, Passed or Failed), and the assessment result, shown as a percentage.

Example: This report can be used to ensure that all users in your organisation have completed and passed assessments in topics that are part of mandatory training (for example, things like vital security, data protection, or food hygiene training), which users may be required to complete in order to work safely and securely at your organisation or within your industry. You might then follow up with any users who have not successfully passed those assessments to ensure that they do. You might also generate this report as proof that all required users in your organisation have passed the assessments and then share it with key members of your organisation, such as management, trainers, auditors, or heads of department so that they are made aware.

  • The list below does not include any custom profile fields that may be included in the report that are specific to your organisation or fields that can be added optionally during creation. For example, your organisation may have custom fields such as Location, Department, or Job Title that can be included in this report.

The unique identifier of the user who has access to the topic with the formal assessment.

Start date

The date the user started the formal assessment. This is as soon as the user clicks Start assessment and views the first question in the formal assessment.

Completed date

The date and time the user passed/completed the formal assessment.

Assessment user status

The user’s status for the formal assessment.

The available statuses are:

  • Started: The user has started the formal assessment but has not yet answered all the questions.

  • Passed: The user has passed the formal assessment by answering all the questions in the formal assessment correctly.

  • Failed: The user has attempted the formal assessment by answering all of the questions in it but has not answered the required number of questions correctly and has therefore failed.

Attempt id

The unique identifier for the user’s formal assessment attempt.

Raw score

The total number of points the user has scored in their latest attempt.

This score is weighted by the platform and not manually by the formula set up in the formal assessment settings.

Assessment result

The user’s assessment result, shown as a percentage. For example, if the user has answered all the questions correctly, 100% is shown in this column for that user.

Optional fields that can be included in the report

The fields in the Optional fields section are optional and are not included in the report by default. You might add some of these fields so that the report includes more detail. The fields listed below do not include any organisation-specific custom profile fields that may be present in the Optional fields section in your organisation’s version of the platform.

Attempt count

The number of completion attempts the user has made for the formal assessment. For example: 1 is shown if a first attempt has been made, 2 is shown if a second attempt has been made, and so on.

The total number of questions presented to the user in the formal assessment.

Nr of correctly answered questions

The total number of questions in the formal assessment that the user has answered correctly.

Nr of incorrectly answered questions

The total number of questions the user has answered incorrectly in the formal assessment.


If the Assessment user status for the user is 'Started' this column will show as N/A.
